APPOINTMENTS

Our office hours are Monday through Thursday 8:00 am to 5:00 pm and Friday 8:00 am to noon. We see patients by appointment only so that we may provide focused and attentive care during your individual appointment time. If you have an urgent need, our office staff is happy to provide recommendations for acute care settings, but we are unable to accommodate walk-ins.

NEW PATIENT CHECKLIST

1. Forms
Please complete and submit all paperwork before your appointment. All paperwork must be submitted to our office at least 7 days prior to your appointment.

2. Medications
Please bring a list of all your current medications (including over the counter medications, supplements, and herbal medications) with you to your first appointment. Some patients find it easiest to bring their medication bottles to the first appointment.

3. Contact Information
Please bring contact information for any family members or emergency contacts that you would like for us to have on file. Please complete a "release of information" form for your primary care provider, your therapist (if you have one), and any other providers you would like for us to be able to communicate with to assist in providing you with optimal care.

4. Payment/Insurance
Please bring your insurance card with you to the first appointment. If we are out-of-network for you, payment in full for services will be collected at the time of service, but we can provide you with a receipt to submit to your insurance provider for reimbursement at your request. If we are in-network, co-pays and deductibles will be collected at the time of service.

5. Cancellations/No-shows
Please review our policies before your first visit. It is our policy that if you need to cancel an appointment, you must give at least 24 hours' notice so that we have the opportunity to offer the appointment time to another patient. If an appointment is missed or canceled twice with less than 24 hours’ notice, further services may be denied.

urine drug testing POLICY

The Mississippi Board of Nursing requires that all patients (including children) who are being prescribed certain controlled substances be administered a point of care urine drug screen at least every 90 days. We utilize urine drug testing to comply with this, as well as federal requirements, and to ensure that patients are using medications safely and responsibly. All disputed or unexpected test results may be sent out to a lab for confirmation at the patient’s request.
